After meeting up these conditions, you will be eligible to apply for the MMJ card and for this, you have to perform some steps, they are-
● Step 1: Firstly, you need to take the appointment of a licensed physician and make a discussion with him about the possibility of having an MMJ Card in Illinois. After considering your condition he will fillup a Physician Written Certification Form in favor of you and send it to the Division of Medical Cannabis under the department of Public Health in Illinois.
